How do I update the name/address/phone number on my documents?


The information in your MyVetLink account and on your certificates comes directly from your veterinary clinic. 


You must contact your veterinary clinic if any changes or updates need to be made to your identifying information appearing on certificates. Your veterinarian can update your contact information at any time. However, completed certificates cannot be edited. The veterinarian will have to void and recreate the certificate to reflect any changes to your name, street address, or phone number.
